The About page on this site already says:
If you believe something on this site is inaccurate, we want to know. Corrections to the underlying record have to go through CMS, but if we have misread or mislabeled what CMS published, that is ours to fix.
This page is how you do it.

Anyone can use this page. You do not have to be the home. Families, staff, state officials and residents' representatives are all welcome to point at something that looks wrong.
First, the important split
There are two kinds of wrong, and they go to two different places.
Kind one: the federal record itself is wrong. The number of findings, the severity letter, the survey date, the fine amount, the star rating, the staffing hours, the bed count, the address, the phone number. All of that comes from the Centers for Medicare & Medicaid Services. We publish what CMS published. We cannot change it here. If we edited a figure by hand, the next monthly refresh would overwrite our edit and the page would go back to the CMS value within weeks. Worse, the site would then be quietly disagreeing with the federal record with no way for a reader to tell.
Getting that corrected means getting CMS to correct it. See the next section.
Kind two: we got something wrong. Our plain-English rewrite of a rule says something the rule does not say. A calculation is off. A label is attached to the wrong thing. A sentence contradicts the chart beside it. A page says "no serious findings" when the record shows some. A home's page shows another home's data. All of that is ours, and we will fix it.
Every one of those is a mistake this site has made and fixed at some point, and there are automated checks in place for each of them now, run before anything is published. That is not a guarantee against the next one. It is why the address below exists.
How to get the federal record corrected
We are not CMS and we cannot speak for CMS, so treat this as a pointer rather than instructions.
The inspection findings on this site are recorded by state survey agencies on behalf of CMS, and the data is published through the CMS Provider Data Catalog at data.cms.gov. A home that believes an inspection finding, a penalty, a staffing figure or a rating is recorded incorrectly normally raises it through the process CMS and its state survey agency already provide, including the formal channels for disputing a survey finding.
If you are a resident's family rather than the home, that same agency is still where to start. Every inspection on this site was carried out by the survey agency of the state the home is in, usually a division of that state's health department, and that agency holds the record CMS publishes. Look for the survey agency, or the licensing and certification division, in the state where the home is, not in the state you live in.
We do not print a phone number or a form address here on purpose. Those change, and sending somebody to the wrong office costs them more than sending them nowhere. Your state's health department will have the current one.
Once CMS publishes the correction, this site picks it up on its own. We reload the federal files after each monthly release, which CMS issues on the last Wednesday of the month. You do not need to tell us twice.
If you want, tell us anyway. If we know a figure is under dispute we can look at whether our wording around it should say so.
What we will fix ourselves, and quickly
- A plain-English rewrite that says more, or less, than the rule it describes.
- Any figure we calculate rather than copy, including the level of care a home's residents would be expected to need and the gap we report against it.
- A label, pill, heading or icon attached to the wrong record.
- A sentence that contradicts a number or a chart on the same page.
- A rounding, unit or date-range mistake.
- A home's page showing the wrong home's data, or the wrong address, if the fault is in how we matched the records rather than in the CMS file.
- Anything on the About page or the How ratings work page that is not true.
- A sentence anywhere on the site that promises something the site does not actually do.
- A figure presented as complete when the federal file it came from only covers part of the record. Fines are the case we know about: the federal penalties file covers roughly the last three years, not a home's whole history.
What to send us
The more of this you can give us, the faster it gets fixed.
- The full web address of the page.
- The exact sentence or figure you are disputing. Copy and paste it.
- What you believe is correct.
- Where that comes from, if you have it. A CMS record, a survey document, a state agency letter, or a corrected file all help.
- How to reach you, if you want an answer.
You do not need all five. Send what you have.
What happens next
- We read it. We read everything that comes in, and we reply once we have checked it. We would rather answer you properly than quickly, so we do not promise a turnaround we might miss.
- We check the claim against the CMS file we loaded and against the code that built the page.
- If it is ours, we fix it and it goes live at the next publish.
- If it is CMS's, we say so plainly and tell you what we found in the file, so you know exactly what to take to CMS.
- Either way we tell you what we did.
What we will not do
We will not take a home's page down because the home dislikes it. The findings on this site are the federal government's published record of a Medicare-certified facility. Removing a page because the record is unflattering would make the site useless to the families it is for.
We will not add or remove a finding on request. Only the federal record can do that.
We will not accept payment to change, feature, or remove anything. No home can pay to change what its page says. There is no advertising from any facility or operator on this site.
If you want to add context
A home may have fixed a problem, contested a finding, or changed hands since the inspection. We do not currently publish statements from homes, and we do not want to start printing marketing copy next to inspection findings.
Where Medicare's own file records that a home is formally contesting a finding, the page says so beside that finding, and that is the only response we print. If you believe a figure is wrong rather than merely unwelcome, the routes above are the ones that change the record.
